Patient Navigator and Care Manager

Cassie has been at PRISM for over 5 years. In addition to hands-on clinical care Mondays and Tuesdays, she now supports our Physicians with care coordination and other offerings remotely Wednesdays, Thursdays and Fridays.

She offers care coordination, accomodations, and chart review appointments in addition to telehealth for postural and ergonomics evluations and recommendations. She provides skills training ordered by our Physicians to educate patients and families on how to perform specified techniques and gain independence at home.
