How to Utilize PRISM

PROVIDER SELECTION:
Read about each of our physician/nurse practitioner staff members and learn about their areas of expertise and treatment styles.
APPOINTMENT LENGTH:
25 min of appointment time per issue for straightforward concerns such as:
Diagnosis and management of joint injury
Evaluation for neck instability
Evaluation for MCAS
Evaluation of headache or concussion
50 min of appointment time for each complex evaluation such as:
Diagnosis evaluation for Ehlers-Danlos Syndrome
Evaluation for longstanding fatigue or Long COVID
Complex pain syndromes
Physician evaluation for possible procedure including ultrasound evaluation
Physician evaluation for Osteopathic Manual Medicine
Biomechanics Evaluation and Treatment Plan
Please don’t feel you must address everything in the first visit! You may choose to do multiple visits to tackle one or two concerns at a time. Whatever your preference may be, we want you to leave our office educated and empowered.

Operative Reports
Imaging Reports AND CDs (or logins to view images)
Recent Labs
Most recent 1-3 doctor notes relevant to discussion
Confirm your appointment by paying the deposit*

Enter your medications and medical history

What Will We Ask?
For straightforward appointments about a single issue or injury, we will want to understand how it happened, timeline of symptoms, what treatments have been tried and if they had any impact, and how the current symptoms are impacting your life.
For appointments dealing with chronic issues or genetic diagnoses, we may also ask more detailed questions about:
Birth history and developmental milestones
Family members with unusual conditions or short lifespans
Childhood, adolescent, and young adulthood health and injury history including subluxations and missed school/work
To help us focus our evaluation and tailor our approach to your needs, we may ask:
1-3 Goals for your Quality of Life as it relates to your health. Exs:
I want to travel
I want to sleep through the night
I want to beat my personal record on the 500m
I want to be able to sit at a desk upright for 3 hrs

PRISM bridges the gap between what is available and what is needed.
To help provide care from professional athletes and Olympians, to persons living with connective tissue disorders, chronic pain, and acute injuries.
PRISM may be utilized as
* an occasional resource when additional expertise is required regarding a specific diagnosis or clinical question
* your "hub" with referrals and care coordination predominantly originating from our office, to meet all of your rehabilitation, regenerative medicine, pain management, and medical care needs for the conditions we treat
* a temporary stop along the way to understanding how to better utilize resources more readily available in your community
We offer
• Virtual peer-to-peer consultations with your outside providers to review findings and explain your care plan
• Care coordination sessions to collaborate with your broader care team and align on next steps for your health
* Virtual ATC health coaching for injury prevention, postural training, vagal nerve exercises and other autonomic/POTS/fatigue-targeted health coaching
Our rehabilitation team can be utilized to
* Create a program for you to bring to outside physical therapists or trainers.
* Virtually join a session with one of your physical therapists to guide cues and help you ensure safety precautions and proper technique
* Take over your rehabilitation for a period of 1-12 weeks and then "graduate" you back to in network PT
* Strengthen and prepare your body ahead of a procedure or surgery to improve healing and outcomes.
* Post-operative recovery
* Provide Dysautonomia/POTS and Vagal Nerve Stimulation and care
* Provide performance coaching for dancers and athletes
Many of these services are reimbursable by Insurance
